Ladies and gentlemen,

 

in connection with the announcement regarding the termination of the Bitmarket exchange, which appeared on the website www.bitmarket.pl on July 8, 2019 and the loss of access to deposited funds by users of the exchange, we would like to inform you that every victim can submit a notification about criminal offense in the organizational unit of the Police or the Public Prosecutor’s Office competent for his place of residence. The notification form will be available for download free of charge at https://cgolegal.pl/ tomorrow from 15 o’clock.

 

We would like to kindly inform you that the Law Firm is also preparing a notification about the possibility of committing a crime with a list of victims. Both the preparation of the notification and its submission are free. Victims who are willing to be included in the notification are asked to send the following data:

 

  1. name and surname;
  2. address of residence;
  3. login to the Bitmarket stock exchange;
  4. the amount of funds lost (in PLN and BTC);

 

along with the following statement regarding the processing of personal data:

 

“I consent to the processing of my personal data provided in this e-mail by CGO Legal Chajdas Gawlak Owczarek sp. K. With its registered office in Łódź (Łąkowa 3/5 street, room 2, building 14, 90-562 Łódź, for which the District Court for Łódź – Śródmieście in Łódź, XX Commercial Division of the National Court Register maintains registration files under KRS number: 0000513817, REGON: 101335024, tax identification number: 7272785705, e-mail address: biuro@cgolegal.pl, contact number (+48) 42 663 65 03) in order to submit a notice about the possibility of committing a crime to my detriment in connection with the termination of the Bitmarket market and loss of access to funds accumulated in my stock exchange account. I was informed of the right to inspect my personal data, correct them and the possibility of withdrawing consent to the processing of personal data at any time.

 

I declare that I acknowledge that:

1) the administrator of the collected personal data is CGO Legal Chajdas Gawlak Owczarek sp. K. With headquarters in Łódź (Łąkowa 3/5 street, room 2, building 14, 90-562 Łódź, for which the District Court for Łódź – Śródmieście in Łódź, XX Economic Department of the National Court Register keeps records under KRS number: 0000513817, REGON: 101335024, tax identification number: 7272785705, e-mail address: biuro@cgolegal.pl, contact number (+48) 42 663 65 03);

2) my personal data will be processed only in order to submit a notice about the possibility of committing a crime to my detriment in connection with the termination of the Bitmarket market and loss of access to funds accumulated on my stock exchange account;

3) providing data is voluntary, however, the refusal to provide it is equivalent to the fact that the Administrator cannot use the notice of the possibility of committing a crime to my detriment;

4) the given data will be stored only for a period of time until it is sent to me to the email address from which the notification was sent, information on the submission of a notification about the possibility of committing a crime, then it will be deleted;

5) I have the right to access my data, correct it, remove or limit processing and withdraw consent to its processing at any time. ”

In response to a notification containing the aforementioned data and a declaration, a confirmation of acceptance of the notification will be sent along with the following information regarding the protection and processing of personal data:

 

“According to art. 14 par. 1 and par. 2 of the General Regulation on the Protection of Personal Data of 27 April 2016, I inform that:

1) the administrator of your personal data is CGO Legal Chajdas Gawlak Owczarek sp. K. With headquarters in Łódź (Łąkowa 3/5 street, room 2, building 14, 90-562 Łódź, for which the District Court for Łódź – Śródmieście in Łódź, XX Commercial Department of the National Court Register maintains registration files under KRS number: 0000513817, REGON: 101335024, tax identification number: 7272785705, e-mail address: biuro@cgolegal.pl, contact number (+48) 42 663 65 03 (hereinafter referred to as “APD” );

2) Your personal data will be processed in order to submit a notice about the possibility of committing a crime for your loss in connection with the termination of the Bitmarket market and loss of access to funds accumulated on my stock exchange account, pursuant to art. 6 par. 1 a) Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of individuals with regard to the processing of personal data and on the free movement of such data and repealing Directive 95/46 / EC (GDPR)

3) the recipient of your personal data will be the Olsztyn-Południe District Prosecutor’s Office in Olsztyn;

4) Your personal data will not be transferred to a third country or an international organization.

5) Your personal data will be stored until it is sent to the reporting person to the email address from which the notification was sent, information on the submission of a notification about the possibility of committing a crime, then it will be removed;

6) you have the right to access your personal data, the right to request correction, deletion or limitation of processing of such data, the right to data transfer;

7) for reasons related to your special situation, you have the right to object to the processing of data, without giving any reason you are entitled to object when data is used for direct marketing purposes;

8) you have the right to withdraw your consent at any time, which, however, will not affect the lawfulness of the processing, which was made on the basis of consent before its withdrawal;

9) you have the right to lodge a complaint with a supervisory authority when you feel that the processing of personal data concerning you violates the provisions;

10) providing your personal data by you is voluntary;

11) Your data will not be processed in an automated way, including in the form of profiling. ”

 

The email address from which the application will be sent will also be sent information about submission of the notification of the possibility of committing a crime.
